Advisor/Committee Member Change Request
This form is submitted any time the student requests a change in advisor or committee member, at any stage in the program.
This form is submitted once a date has been set for your dissertation proposal meeting or final dissertation defense. (Note: for room requests outside of the dissertation, please email PostSecondaryDoc@txstate.edu.)
