If you have received notification that you have been suspended and you wish to appeal your suspension, please fill out this form. You must complete and submit this form in order to be considered for a suspension appeal appointment.
This form should ONLY be submitted by students who were declared with a major in the College of Education at the time of being placed onto academic suspension. DO NOT use this form if your major falls within a different college. Instead, review that specific College’s webpage and complete their appeal process.

Suspension Appeal Forms will only be accepted for Summer Term II 2026 Reinstatement until Thursday, 6/18/26.

Once you submit the Suspension Appeal form, you will be taken to a confirmation page where you must click on the Booking link to SELECT AN AVAILABLE APPEAL APPOINTMENT. These appeal appointments are limited and time-sensitive, so please schedule your meeting ASAP.
