If you have received notification that you have been suspended and you wish to appeal your suspension, please fill out this form. You must complete and submit this form in order to be considered for a suspension appeal appointment.
This form should ONLY be submitted by students who were declared with a major in the College of Education at the time of being placed onto academic suspension. DO NOT use this form if your major falls within a different college.
If you have been suspended and are seeking reinstatement for an upcoming term, you must complete the Suspension Appeal Form during the designated timeframe listed here:
Suspension Appeal Form will only be accepted for [Summer I Reinstatement between Tues 5/13/25 and Thurs 5/15/25] BOLDED
Suspension Appeal Form will only be accepted for [Summer II Reinstatement between Tues 6/10/25 and Tues 6/17/25] BOLDED
Suspension Appeal Form will only be accepted for [Fall Reinstatement between Tues 8/5/25 and Tues 8/12/25] BOLDED
Suspension Appeals cannot be accepted or processed outside of these dates.
Once you submit the Suspension Appeal form, you will be taken to a confirmation page where you must SELECT AN APPEAL APPOINTMENT DATE CORRESPONDING TO THE SEMESTER YOU ARE REQUESTING REINSTATEMENT. These appeal appointments are limited and time-sensitive (especially for Summer I), so please schedule your meeting ASAP. If you select an appointment date for a different semester, it may be too late to process your appeal, so please select carefully.
