New Hire Forms

Send an email to Rebecca Torres and include your A#, date of birth and the project name (or name of faculty member).

Please complete the steps below. Deliver forms to ED 2134 (located inside the COE Advising Center) or scan and email them to Rebecca Torres. Include your A# and date of birth in your email.

  Step 1: Complete these required forms

• Hourly Student Worker Acknowledgement Form
• W-4 Form

  Step 2 (optional): Options you may select

• Election for Online W-2 Enrollment
  • Allows you to opt into accessing your annual W-2 form online rather than having it mailed to you
• Direct Deposit Authorization
  • Allows you to have your paychecks directly deposited into a designated bank account.

  Step 3: Review Required Notices

• Please review the list of required notices for new employees of Texas State.

  Step 4: Collect original identification (I-9) documents and schedule a time to meet with Rebecca Torres.

  Step 5: Add your Identification Documents

• You will receive an email from HireRight Customer Support (noreply@hireright.com) and will be required to add your identification documents.
